Axis Primary Care Clinic delivers longitudinal healthcare for our patients. Our team is comprised of nurse practitioners, registered nurses, a social worker, and a mental health clinician, working collaboratively to provide quality primary care services to the Surrey and North Delta, BC community. It is our goal to optimize patient health and wellness, using a team-based approach through collaboration with other healthcare professionals and community services.

Axis Primary Care Clinic provides services on the land of the Coast Salish Nations and within the shared ancestral, traditional and unceded territory of the Semiahmoo, Kwantlen and Katzie Nations.
